Does anyone know what Ontario, specifically the City of Toronto, requires to pass vehicle inspection?

Things like mud-flaps, wider flares, quiet muffler, full-width bumpers, third brake light, daytime running lights, etc? Also, if wider flares are required, do they have to cover the whole tire (side bulge included) or just the tread?

Is there a height restriction/limitation?

The requirements for vehicle certification are standard across all of Ontario.

It is a general mechanical inspection. Most of the things that you listed are covered in the Highway Traffic Act, but do not affect certification. There are no height restrictions.

Toronto will also require emissions testing, as will most areas in Ontario.
